AbAbout COVID-19 Vaccination Certificates (Vaccination Passports) for Overseas Travel

We are now accepting applications for COVID-19 vaccination certificates (vaccination passports) from Shinagawa residents with plans to travel overseas. As a rule, these vaccination passports are issued the same day the application is received.

Eligibility
You must meet all of the following conditions:
1. Have a plan to travel overseas
2. Have a valid passport
3. Have undergone vaccination using a Shinagawa City vaccination ticket

Note: On the application form you must write the name of the country/countries you plan to travel to and your scheduled dates of travel.

Required Documents and Application Procedures
● Those file an application at a service counter
Required documents:
1. Passport
2. Vaccination ticket (vaccination completion certificate) or vaccination record

Service counter locations and hours (closed on national holidays)
• Family Registry and Residents Section (Hon Chosha 3F)/Ebara Dai-ichi Community Center
Sunday through Friday, 8:30 a.m. to 5:00 p.m. (Tuesdays until 7:00 p.m.)
• Community Centers: Monday through Friday, 8:30 a.m. to 5:00 p.m.
• Oimachi or Shinagawa City Meguro Administration service corner: Monday through Friday, 8:00 a.m. to 7:00 p.m. (Sundays, 10:00 a.m. to 3:00 p.m.) * Closed on Saturdays.

● Anyone Who Files an Application by Postal Mail
Required documents:
1. Application form
2. A copy of your passport
3. A copy of your vaccination ticket (vaccination completion certificate) or vaccination record
4. A return envelope (no need to affix a stamp)
5. A copy of some form of official personal ID such as a driver’s license or individual number card with your address where the passport should be mailed written on it.

Note: The application form is available at the abovementioned service counters, and can also be downloaded from the Shinagawa City website.
Mailing procedures: Write “COVID-19 Vaccination Certificate” in red on the front of the envelope and your return address on the back. Place the required documents in the envelope, and then send it to the Unit in charge of Vaccine Certificate of the Family Registry and Residents Section (Shinagawa City Office 140-8715).

The following people must present additional required documents when filing an application, either in person at a service counter or by postal mail.
● Anyone with a maiden name, different last name, or different first name (in English) on his/her passport
➡ A copy of some form of official personal ID with the maiden name, different last name, or different first name (in English) written on it such as a driver’s license or individual number (My Number) card with the maiden name, etc. also written on it.
● Anyone filing by using a proxy
➡ A proxy letter
Note: Even members of the same household must present this letter.



About COVID-19 Vaccinations

● An Announcement Concerning the AstraZeneca Vaccine
Shinagawa City will begin providing the AstraZeneca vaccine to the following people.

Eligibility:
As a rule, those who will be 40 years of age or older on the day of vaccination (18 years of age or older for those with special needs).

【If You Have Special Needs】

(1) Those who are unable to receive the mRNA vaccine (such as Pfizer or Moderna) due to PEG (polyethylene glycol) allergies.
(2) Shinagawa residents who have completed one shot of this vaccine overseas.

About Vaccination Completion Certificates

Residents who have been vaccinated but do not plan to travel overseas can still show the COVID-19 vaccination completion certificate (temporary) (the dotted area in the illustration below) to verify that they have undergone vaccinations, so please keep your vaccination ticket (which includes the certificate) even after being vaccinated.
