National Health Insurance Premiums for FY2016 Have Been Determined

National Health Insurance premiums for April 2016 through March 2017 are the sum of the basic portion (for medical treatment), the latter-term elderly support portion, and the Nursing Care Insurance premiums (for those between 40 and 64 years of age).
The per capita amount and insurance premium rates for income-based amounts are determined based on the amount of medical benefits covering medical expenses, the support portion for Latter-Term Elderly Medical Insurance, premiums paid by Type 2 members of Nursing Care Insurance, and other factors.

○ Calculation Procedures

Annual insurance premium for a household = Basic portion (for medical treatment) + Latter-term elderly support portion + Nursing Care Insurance premiums
Basic portion (for medical treatment; maximum ceiling: 540,000 yen) = Income-based amount (standard leviable income* of all household insurance members × 6.86%) + Per-capita amount (35,400 yen × No. of insurance members in the household)
Latter-term elderly support portion (maximum ceiling: 190,000 yen) = Income-based amount (standard leviable income* of all household insurance members × 2.02%) + Per-capita amount (10,800 yen × No. of insurance members in the household)
Nursing Care Insurance premiums (maximum ceiling: 160,000 yen) = Income-based amount (standard leviable income* of all household insurance members between 40 and 64 years of age × 1.35%) + Per-capita amount (14,700 yen × No. of insurance members in the household between 40 and 64 years of age)
*Standard leviable income = Total income and so on 330,000 yen (basic deduction)

○ FY2016 Payment Months and Calculation Period
The premiums for the year will be announced in June, and will be divided into ten installments to be paid from June through March. No payments are made in April or May.
Mid-June: Insurance premiums notification and six invoices (for June–October the first-half payments) are mailed out.
Mid-November: Five invoices (for November–March payments) are mailed out.
Note: Households that pay via account transfer will only receive notification of the premium amounts.

○ If you have not reported income, please do so for the municipality in which you resided as of January 1. Insurance premiums cannot be reduced unless each and every member of the household files an income report, even if the incomes of all household members are low.
Note: If you moved into Shinagawa City on or after January 2, the city office will contact the government of the municipality where you lived previously about your income. At first, you will be notified of the per capita amount only. Once your income has been confirmed, you will then be sent a notification of adjusted premiums.

- Employment insurance members 64 years of age or younger who lost their jobs involuntarily on or after March 31, 2013, must apply to have their premiums reduced.
- Insurance premiums will be deducted from the head of household’s pension payments (special collection) between 65 and 74 years of age when all of the following apply:

  1. The head of household is a National Health Insurance member
  2. All the National Health Insurance members in the household are between 65 and 74 years of age
  3. The annual pension amount is 180,000 yen or more, and the sum of National Health Insurance premiums and Nursing Care Insurance premiums does not exceed half of the pension amount
