About Your Individual Number
(“My Number”)

● If You Did Not Receive Your Notification Card
Notification cards were sent by registered mail to registered residents of Shinagawa City as of October 5, 2015. A postcard was sent to those who have not yet picked up their cards with instructions to pick up them at the service counter within three months. Cards will be issued at the service counter until June 30. If you have not received your notification card, please do so before the deadline. For notification cards that came back to the city office in or after April please pick up them by the deadline written on the postcard.

To pick up your notification card at the city office
Location: Notification Card Receipt Service Counter (Shinagawa City Office Dai-ni Chosha 3F)
Hours: Monday through Friday, and Sunday, 8:30 a.m. to 5:00 p.m.
Notes:
1. Until 7:00 p.m. on Tuesdays
2. Until 7:00 p.m. on weekdays until September 29 (Wed.)
What to bring: The information postcard you received and some form of official identification, such as a driver’s license or passport
Note: If you do not have the information postcard, please bring the postal delivery slip you received in your absence (if you have it).

○ Please call for instructions if someone other than you or someone in your household member will pick up the card on your behalf.
Note: If your employer or other entity asks you to show your notification card and if you are in a rush, you can use a copy of your resident record (juminhyo)—which has your individual number written on it—in lieu of the card. (There is a fee for copying your resident record.)

○ Caution
Beware of any improper solicitation or attempts to acquire personal information taking advantage of this new system. If you receive any suspicious telephone calls, e-mails, letters or visits having to do with the system, please call the “My Number” general free dial service.

● Procedures for Individual Number Notification Card or Individual Number Card with a Change of Address

In This Case Type of Notification Form Card Procedures
When you move into Shinagawa City from another municipality Notification of moving in The address on the card must be changed.
When you move within Shinagawa City Notification of change of address
When you move from Shinagawa City to another municipality Notification of moving out (within Japan) There are no procedures required for your card when filing notification of moving out.
When you move from Shinagawa City to another country Notification of moving out (overseas) You must file a card return notification form.

○ If you change the address on your notification card, you will not be able to use the individual number card application form enclosed with your notification card because it has your old address. You must ask for a new application form at the service counter to apply for an individual number card.

● If You Lose Your Notification Card
If you lose your notification card, you can apply for a reissue (you will be charged a fee for this). Please submit a lost item report with the police, and bring your lost item report receipt number and some form of official personal ID to the service counter to apply for a new card. If someone else files an application on your behalf, he/she will need to present a proxy letter and some form of official ID.
